One of the most challenging aspects of knowledge management is the harvesting, analysis, and synthesis of information in a transferable form. The USAID Health Care Improvement Project has risen to this challenge by examining current practices to harvest, analyze and synthesize information, and developing a set of tools to be used by QI teams and coaches to facilitate this knowledge management process, based on best practices and innovating to address current gaps. The resulting templates/tools form the project's "Standard Evaluation System (SES)". The system is “standard” because the tool templates provide a common set of key information to be collected and processed. It addresses “evaluation” because it focuses on compiling and analyzing data that answer the question – “What changes lead to improvement?” It is a “system” because the series of tools are designed to facilitate documentation, analysis, and synthesis processes at several levels (QI team, collaborative, country, international) as part of a knowledge management system.
