The Synthesis form is designed to capture information for sharing with other teams. The Synthesis form summarizes key changes the QI team has concluded worked (or did not work) and that they feel are worth sharing with other teams.  The essential features of this form include justification that the change is an improvement, description of steps to implementing the change, and information about overcoming barriers. Here, the level of detail is determined by what other teams need to know. As designed, the synthesis form can be completed by QI teams alone or with assistance from coaches.  

The synthesis form includes four sections: 1) one in which teams outline what they are trying to accomplish (improvement objectives and indicators); 2) a section on implemented changes they want to share (both effective and ineffective changes) with information on what is needed to implement these changes; 3) a graph template for demonstrating and annotating results of these changes; and 4) a place to share other relevant information.
